Having a stoma or an ileal pouch is a life-changing chronic condition that requires significant rehabilitation and adaptation to ensure the ostomates’ and pouch patients’ quality of life is restored. The presentation explores the unmet needs of these patients in the community, focusing on the insufficient support they receive post-discharge from the hospital. Many patients report feeling abandoned during the critical first 3-6 months, a period marked by the highest readmission rates. Studies indicate that over 40% of patients are readmitted 3-4 months post-operatively. These findings highlight the urgent need for enhanced community support systems to improve patient outcomes and reduce readmission rates. To mitigate this, our nursing service offered support 7 days a week, including evenings and bank holidays, with out-of-hours and rapid response care. The preliminary results suggest that this comprehensive support improves patients’ adaptation to life after surgery and prevents hospital readmissions.
